To participate in this trial, you must be 65 years or older and have been hospitalized for heart-related issues. It's important that you're expected to go home after your hospital stay. Unfortunately, if you have difficulty understanding or managing your health without help, or if you're receiving other medical services that could conflict with this program, you may not be eligible. If you join, you can expect to receive personalized support from a nurse, which could help improve your health and confidence in managing your heart condition.

Eligibility criteria
- Inclusion Criteria:
- • aged 65 and over
- • hospitalized for cardiac decompensation, regardless of ejection fraction (FE) value (preserved or decreased)
- • expected to be discharged home will be considered eligible.
- Exclusion Criteria:
- • Individuals who lack the cognitive and/or physical capabilities (Mini-COG +) for self-monitoring of vital signs, and without a caregiver available to assist them
- • people who receive other medical services
