This clinical trial is exploring a new program called MultiComponent TeleRehabilitation (MCTR) designed to help older Veterans who have multiple health issues and struggle with physical activity. The goal is to see if this program, which offers high-intensity rehabilitation, self-management training, and social support through technology, can improve their strength and physical function better than traditional rehabilitation methods that usually require in-person visits. This is important because many older Veterans experience physical decline, loneliness, and a lower quality of life.
To participate in this trial, individuals need to be 60 years or older, have multiple chronic health conditions, and show some difficulty with physical activities, as measured by a simple sit-to-stand test. However, those with certain severe health issues or a life expectancy of less than 12 months won’t be eligible. If you join the trial, you can expect to take part in a program that combines exercises with support from healthcare professionals, all from the comfort of your home. Eligibility criteria
- Inclusion Criteria:
- • 60 years of age and older
- • Multiple chronic conditions (Functional Comorbidity Index \> or = 3)
- • Impaired physical function (\< or = 8 repetitions on 30 second sit to stand test)
- Exclusion Criteria:
- • Life expectancy \< 12 months
- • Acute or progressive neurological disorder (e.g. amyotrophic lateral sclerosis, recent stroke)
- • Moderate to severe dementia (\<11 on telephone Montreal Cognitive Assessment(T-MoCA))
- • Medical conditions precluding safe participation in high-intensity rehabilitation (e.g. unstable angina)
