This clinical trial, called "A Socio-ecological Approach for Improving Self-management in Adolescents With SCD," is studying a program named SCThrive to see how it can help teenagers and young adults with sickle cell disease (SCD) manage their health better. The trial will involve 260 participants aged 13 to 21 who are receiving care at specific clinics. Researchers want to find out if SCThrive can increase participants' confidence in managing their condition, help them take better care of themselves, improve their daily activities, and reduce the number of times they need to visit the emergency room.
To be eligible for this trial, participants must have a confirmed diagnosis of SCD and be patients at one of the participating clinics. They will take part in an 8-week virtual group program using the SCThrive mobile app, and their progress will be checked through surveys before, right after, and three months after the program ends. The researchers will compare the outcomes of those using SCThrive with those receiving standard care, which includes educational messages about SCD. This study is currently recruiting participants, and it offers a valuable opportunity for young people with SCD to improve their self-management skills and overall well-being.

Eligibility criteria
- Inclusion Criteria:
- • Patient of a participating SCD Clinic
- • Confirmed diagnosis of SCD
- • 13-21 years of age
- Exclusion Criteria:
- • Another chronic disease (which would complicate measurement of patient activation)
- • Non-English-speaking
- • Cognitive or psychiatric disorder that the physician or study therapists believe would impair study participation.
