This clinical trial is exploring a new program designed to help people who have survived serious injuries. The main goal is to see if this program effectively meets the medical and social needs of these survivors. Specifically, the researchers want to find out if the program is suitable, acceptable, and practical for those who have gone through traumatic injuries.
To participate, individuals must be at least 18 years old, have been treated for a physical injury at Zuckerberg San Francisco General Hospital, and stayed in the hospital for at least 24 hours. They should also be residents of San Francisco or currently without a home in the city. Participants will be asked to take part in detailed interviews and share their experiences using the program’s resources. This study is currently not recruiting participants, but it aims to improve support for injury survivors in the future.

Eligibility criteria
- Inclusion Criteria:
- • Patient ≥ 18 years old presenting with a physical injury to Zuckerberg San Francisco General Hospital
- • Admitted to the hospital ≥ 24 hours
- • Discharged from the hospital alive
- • San Francisco resident or unhoused in San Francisco
- • Capacity for informed consent
- Exclusion Criteria:
- • Patient \< 18 years old
- • Deceased during index hospitalization
- • Unable to provide informed consent
