The AI4HOPE Dementia Care Planning Pilot Study 2 (AI4HOPE2) is a research study designed to test a new digital tool that helps with care planning for patients with dementia. This tool aims to support both patients and their families in making important decisions about care. The study is not currently recruiting participants, but when it begins, it will focus on individuals with mild to moderate dementia who can either give consent themselves or have a family member or caregiver provide consent for them.
To be eligible for the study, participants need to have mild or moderate dementia and be able to assess their own health or experiences. Unfortunately, those with severe cognitive impairment or other serious health issues may not qualify. If accepted into the study, participants will have the opportunity to use the digital tool and share their experiences, which will help researchers understand how effective it is in supporting dementia care. This study is a step toward improving care planning for individuals living with dementia and their loved ones.

Eligibility criteria
- Inclusion Criteria:
- • Mild or moderate dementia of any type
- • Able to provide informed consent, or consent from surrogate decision maker
- • Able to self-assess patient-related outcome (PROM) or experience (PREM) measures
- Exclusion Criteria:
- • Ongoing delir
- • Severe cognitive impairment (MMSE \< 10 \[22\])
- • Unable to provide informed consent
- • Life expectancy estimated less than 3 month
