This clinical trial is studying how different body positions can affect how well obese patients breathe during surgery when using a facemask for ventilation. Specifically, the researchers want to see if lifting the head and torso of patients by 25 degrees helps them get enough oxygen compared to lying flat on their back. This is important because some patients, especially those with a higher Body Mass Index (BMI), can have difficulty getting enough oxygen through a facemask.

To participate in the trial, patients need to be at least 18 years old, have a BMI greater than 40, and be scheduled for elective surgery with general anesthesia. They should also be able to understand the study and provide consent. However, certain individuals, such as pregnant women or those with specific health conditions, may not be eligible. If you qualify and choose to participate, you will help researchers learn more about the best ways to support breathing for patients with obesity during surgery.

Eligibility criteria

  • Inclusion Criteria:
  • BMI \> 40 kg/m2
  • age \> 18 years
  • scheduled for elective surgery under general anesthesia
  • have the ability to comprehend the rationale for the study and provide consent
  • Exclusion Criteria:
  • pregnancy,
  • risk of aspiration of gastric content
  • patients using glucagon-like peptide(GLP)-1 agonists
  • emergency cases
  • upper airway disease or airway anatomical abnormalities
  • presence of major cardiovascular, respiratory, or cerebral vascular disease
  • if the provider anesthesiologist indicates an awake technique to secure the airway
