This clinical trial is looking at two different surgical approaches for patients who need total knee replacement (TKA) in both knees at the same time due to osteoarthritis, which is a type of arthritis that causes pain and stiffness in the joints. The trial compares "restricted kinematic alignment" (rKA) with "mechanical alignment" (MA) to see which method leads to better recovery and less pain after surgery. Participants will have one knee operated on using rKA and the other knee using MA, allowing researchers to directly compare the results.
To join the study, participants should be between 40 and 80 years old, diagnosed with osteoarthritis in both knees, and healthy enough for surgery. Those with certain conditions, like severe obesity or previous knee surgeries, won't be eligible. If you decide to participate, you can expect to undergo surgery on both knees, and your progress will be monitored to see how well you recover and manage pain. This trial is currently recruiting participants of all genders within the specified age range.

Eligibility criteria
- Inclusion Criteria:
- • Age 40-80 years
- • Diagnosis bilateral primary osteoarthritis of knee joint and undergo simultaneous bilateral total knee arthroplasty
- • ASA classification I-II
- Exclusion Criteria:
- • Valgus deformity
- • BMI \> 40kg/m2
- • History of previous knee surgery
- • Active infection of the knee joint
