This clinical trial is looking at how frailty—when a person becomes weaker and more vulnerable due to age—affects the health outcomes of various chronic diseases, and how those diseases can impact frailty. Researchers will reach out to participants from a previous study (the FRASNET study) to see how their health has changed since they were last evaluated in 2016 and 2017. They will also include patients with mild cognitive impairment or dementia from the Neurologic Unit at the San Raffaele Hospital to gather more information.
To be eligible for this study, participants need to be at least 18 years old and either have previously participated in the FRASNET study or be a current patient with mild cognitive impairment or dementia. Participants will be asked to sign a consent form and will undergo detailed health assessments during their visits. It's important to note that individuals with a life expectancy of less than six months or those with unstable health conditions won’t be able to join this study. This trial aims to better understand the connections between frailty and chronic diseases, which could help improve care for older adults in the future.

- Inclusion Criteria:
- • Having participated in the FRASNET study or being a patient with mild cognitive impairment or dementia followed up at the Neurologic Unit of the San Raffaele Hospital
- • Ability to sign written informed consent
- • \> 18 years old
- Exclusion Criteria:
- • Life expectancy \< 6 months
- • Unwillingness to participate in the study
- • Unstable clinical disease(for ex. serious acute pathologies)
