This clinical trial, called the Food Insecurity Reduction & Strategy Team, is focused on helping people with diabetes who are struggling to get enough healthy food. The study aims to understand the reasons why some patients with diabetes have trouble accessing nutritious food while they are in the hospital. It will also test a special nutrition program to see if it can improve health outcomes for these patients. Participants in the program will receive additional support, including nutritious meals delivered twice a day for up to 90 days after leaving the hospital, as well as education to help them make better food choices.
To be eligible for this trial, participants must be adults aged 65 to 74 who have either Type 1 or Type 2 diabetes, are currently hospitalized at Stanford Healthcare, and have been identified as having food insecurity. They must also live in California and be enrolled in a healthcare plan that covers meals from Mom's Meals. This study is actively recruiting participants and aims to not only improve individual health but also create a sustainable plan to help more people in the community access healthy food in the future.

Eligibility criteria
- Inclusion Criteria:
- • Diagnosis of Type 1 or Type 2 Diabetes Mellitus
- • Admitted to Stanford Healthcare inpatient unit
- • Residence in California at time of enrollment
- • Positive Screening for Food Insecurity
- • On a Healthcare Plan covered by Mom's Meals.
- Exclusion Criteria:
- • Plans to be discharged to a skilled nursing facility.
- • Patients who prefer a language for which a short-form consent is not available.
- • No Home Address
- • Pregnant Participants.
- • No access to refrigerator.
