This clinical trial is studying how exercise can help patients with Chronic Obstructive Pulmonary Disease (COPD) improve their lung health. The researchers believe that a tailored exercise program might reduce harmful changes in the lungs caused by inflammation and stress, potentially leading to better lung function and quality of life. They will also look for specific biological markers in blood, urine, and lung tissue that could indicate how well the lungs are healing and how the body is responding to the exercise.

To participate in this study, individuals must be between 18 and 75 years old and have moderate to severe COPD, as measured by a specific lung function test. They should not have recently experienced any flare-ups of their condition and must be in stable health without other major diseases. Participants will engage in an exercise training program and will have their lung function and overall health monitored throughout the study. This research could provide valuable insights into how exercise can benefit those living with COPD.

Eligibility criteria

  • Inclusion Criteria:
  • Forced expiratory volume in one second (FEV1) 85-30 % of predicted post bronchodilator
  • Optimal medical treatment according to GOLD and the Swedish National Guidelines
  • Absence of other significant diseases that could contribute to physical exercise limitation
  • Non-smoking since at least one year
  • Free from exacerbations (last 4 weeks)
  • BMI = 18- 35
  • Exclusion Criteria:
  • Regular participation in adjusted/specially designed exercise sessions
  • Unstable cardiovascular or pulmonary disease
  • Smoking
  • Any other criteria that at the discretion of the investigator prevent the study subject from participating in an exercise-training program
