This clinical trial is comparing two different surgical methods for treating large ventral hernias, which are defects in the abdominal wall. The two techniques being studied are open ventral hernia repair (OVHR) and a newer, less invasive method called robotically-assisted ventral hernia repair (RVHR). The main goal is to see which method leads to better outcomes, including how long patients stay in the hospital, the rate of complications or issues after surgery, how often hernias come back, and overall quality of life after the procedure.
To participate in this trial, you need to be at least 18 years old and have a hernia that is between 4 and 15 centimeters wide. You also need to be healthy enough for surgery, as determined by your doctor. If you join the study, you will have the surgery and then be followed up for several years to check how you’re doing, with follow-up calls or visits at specific times. It’s important to know that this trial is not yet recruiting participants, and only surgeons with experience in both surgical methods will be performing the operations.

Eligibility criteria
- Inclusion Criteria:
- • Patient age \>18 years
- • Informed consent obtained
- • Transverse diameter of ventral hernia \>4cm - 15cm
- • Eligible to open and minimally-invasive surgery according to preoperative anaesthetic assessment
- Exclusion Criteria:
- • precedent hernia treatment with mesh placement in the retromuscular space
- • precedent anterior or posterior component separation or transversus abdominis release (TAR)
- • active wound infection
- • current cancer diagnosis
- • presence of ileostomy, colostomy or ileal conduit
- • liver disease defined by the presence of ascites
- • end-stage renal disease requiring dialysis
- • need of an emergency surgery
- • pregnancy
- • Criteria for participating surgeons Each participating surgeon has performed 20 or more OVHR and RVHR.
