This clinical trial is studying two different combinations of medications to see which one works better for sedation during painful procedures in the Emergency Department. The trial will include 50 adult patients who are at least 18 years old and need sedation for procedures like fixing broken bones or inserting tubes. Participants will be randomly assigned to one of two groups: one will receive a combination of ketamine and midazolam, and the other will receive ketamine and propofol. Both groups may receive an extra dose of ketamine if they need more sedation.
If you are eligible to participate, you must not have any conditions that would make it difficult to secure your airway, and you should not be pregnant or over 65 years old. The trial is not yet recruiting participants, so it's important to keep an eye out for updates if you or someone you know might be interested. Throughout the trial, the medical team will monitor participants for any complications related to the sedation, ensuring that safety is always a priority.

Eligibility criteria
- Inclusion Criteria:
- * Adult patients (≥18 years) requiring procedural sedation and analgesia for painful procedures in ED . The following procedures are indications for PSA:
- • Cardioversion
- • Reduction of fracture or joint dislocation
- • Procedures as central venous catheter (CVC) and chest tube insertion or wound management
- Exclusion Criteria:
- • Patients with anticipated difficult airway.
- • Post cardiac arrest patients.
- • Contraindication to study drugs.
- • Pregnant women.
- • Patients \> 65 years.
