This clinical trial, called the IDEAL SKIIN CARES Bundle to Prevent Pressure Injury, is studying how effective special training in pressure injury care is at preventing skin problems like bed sores in hospitalized patients. The trial will include participants who are adults aged 18 and older and have been in the hospital for at least 48 hours. To be eligible, participants should not have serious skin conditions or existing pressure injuries when they come to the hospital. Importantly, those who have limited mobility, require special assistance to move, or have certain medical issues will not be included in the study.
Participants in the trial will be divided into three groups. One group will receive training on how to prevent pressure injuries, while another group will receive training on a different topic that doesn’t relate to skin care. The last group will get standard care without any extra training. Throughout the trial, trained nurses will provide care to help prevent pressure injuries. The goal is to see how training affects the number of new pressure injuries that develop during the hospital stay. This research could help improve patient care by identifying the best ways to prevent these injuries in hospitals.

Eligibility criteria
- Inclusion Criteria:
- • be adults aged 18 years or older;
- • have been hospitalized for a minimum of 48 hours;
- • be willing to participate.
- Exclusion Criteria:
- • patients with significant pre-existing skin conditions or injuries that make it difficult to observe skin integrity;
- • have existing PIs or vascular ulcers upon admission to the hospital (pressure injuries defined by the National Pressure Ulcer Advisory Panel);
- • be at risk of PIs due to limited mobility (requiring physical or mechanical assistance to reposition or move);
- • individuals with urinary or fecal incontinence, mental illness, or severe cognitive impairment upon hospital admission;
- • patients who are hospitalized in day-surgery, critical care, emergency, maternity, pediatrics, mental health and dialysis units;
- • admitted to the hospital for \>28 days;
- • those with prior experience of PIs or training in PIs care and prevention.
