This clinical trial is studying a medication called fosaprepitant to see if it is more effective at relieving nausea and vomiting compared to a common anti-nausea medication called ondansetron. The trial aims to find out how well fosaprepitant works and whether it causes any side effects. Fosaprepitant is part of a new class of drugs that targets the brain to help control nausea and vomiting, and it’s thought to be more effective for many patients, especially those who visit emergency departments for these symptoms.
To participate in the trial, you need to be an adult aged 18 or older who is experiencing nausea or vomiting. Certain individuals, such as those who are pregnant or breastfeeding, have used anti-nausea medications before the study, or have specific heart conditions, won’t be eligible. If you join the study, you can expect to receive either fosaprepitant or ondansetron and will be monitored for two hours to see if you need any additional treatment for your nausea or if you experience any new symptoms during that time. This trial is currently recruiting participants, and your involvement could help improve treatment options for others facing similar health challenges.

Eligibility criteria
- Inclusion Criteria:
- • Adults at least 18 years old
- • Present for nausea and/or vomiting as defined by the International Classification of Diseases (ICD-10) or identified by treating clinician
- Exclusion Criteria:
- • Pregnancy, desiring pregnancy, or lactating
- • Antiemetic use or intravenous fluids prior to screening
- • Bradycardia (less than 60 bpm heart rate)
- • Prolonged QTc (\>480ms)
- • Not conversant in English or Spanish
- • Altered mental status
- • Dementia
- • Lack of phone for follow-up communication
