This clinical trial is focused on improving care for individuals with chronic pain who are using long-term opioid medications. Researchers want to see if two specific approaches—Mindfulness-Oriented Recovery Enhancement (which teaches mindfulness techniques to help manage pain and emotions) and patient-centered management visits—can help reduce pain, improve overall quality of life, and lessen the risks associated with opioid use.

To participate in this trial, you need to be at least 18 years old, have been diagnosed with chronic pain for at least three months, and have been using opioids for that same time period. You should also have a moderate level of pain that affects your enjoyment of daily activities and may need to meet certain criteria related to opioid use risks. Participants can expect to receive support and guidance through these new methods and will be actively involved in their pain management plan. If you have a cancer diagnosis related to your pain or certain mental health conditions, you may not be eligible for this study.

Eligibility criteria

  • Inclusion Criteria:
  • Age 18 +
  • Current chronic pain diagnosis (pain for ≥3 months) determined by physician assessment
  • Current use of opioids for ≥3 months
  • Score \>=3 on the Pain Enjoyment General Activity scale
  • Score \>=9 on the Current Opioid Misuse Measure OR lifetime psychiatric or substance use disorder diagnosis OR benzodiazepine use OR history of overdose OR physician determination that risks of opioid use may outweigh the benefits of opioid use
  • Exclusion Criteria:
  • Chronic pain due to a cancer diagnosis
  • Active suicidal intent, schizophrenia, psychotic disorder
  • Prior experience with Mindfulness-Based Stress Reduction, Mindfulness-Based Cognitive Therapy, Mindfulness-Based Relapse Prevention, or Mindfulness-Oriented Recovery Enhancement
