This clinical trial is studying the safety and effectiveness of two different methods for keeping the airway open during surgery in older patients who are at high risk for complications. Specifically, it compares the use of a laryngeal mask airway, which sits in the throat, to traditional tracheal intubation, where a tube is placed down the windpipe. Researchers want to find out if using the laryngeal mask is just as safe as intubation in preventing lung problems after surgery in patients aged 70 and older who are scheduled for certain types of non-heart surgeries.
To participate in this study, patients must be 70 years or older and have a specific score indicating a high risk for complications (known as the ARISCAT score). Those who are having emergency surgery, have certain throat issues, or have other health concerns that might complicate the trial are not eligible. If chosen to participate, individuals can expect to undergo their planned surgery with either the laryngeal mask airway or tracheal intubation and will be monitored for any lung-related issues afterward. This trial is important because it aims to improve care for older patients undergoing surgery, helping doctors understand the best way to keep their airways safe.

Eligibility criteria
- Inclusion Criteria:
- • Patients aged ≥ 70 years scheduled for elective non-cardiothoracic surgery with an ARISCAT score \> 44 will be included
- Exclusion Criteria:
- • Exclusion criteria include emergency surgery; anticipated difficult tracheal intubation; laryngeal pathology that might interfere with supraglottic device insertion; high risk of gastroesophageal reflux; participation in any conflicting trial within the past 3 months; or deemed unsuitable for the trial by investigators.
