This clinical trial is exploring a new way to care for patients with severe liver disease, known as decompensated cirrhosis, through a program called Health at Home (H@H). The goal of the study is to learn from patients about their experiences and opinions regarding receiving medical care at home instead of in a hospital. Participants will go through three phases: first, while they are still in the hospital; second, during the home care program; and third, as they transition to managing their health independently with the support of their caregivers.
To join the study, participants must be at least 18 years old, speak English, and have been diagnosed with chronic liver disease. They also need to have a caregiver available to help them during the first two days after leaving the hospital. Throughout the trial, participants will share their thoughts and feelings about the home care experience, which will help improve future care for patients with liver disease. This trial is currently recruiting participants, and it's important for anyone interested to check the eligibility criteria to see if they can take part.

Eligibility criteria
- Inclusion Criteria:
- • Enrolled into IUH's H@H program
- • At least 18 years of age
- • Chronic liver disease/cirrhosis based on characteristic clinical, laboratory, and imaging findings
- • English speaking
- • Able to provide consent
- • Caregiver able to be present during the acute phase of care (first 48 hours post-hospital discharge)
- • Able to perform activities of daily living independently
- • Lives within IU Health Home service area
- Exclusion Criteria:
- • Unable to complete study questionnaires due to neurocognitive disease, legal blindness or hearing loss
- • Transplant of organ other than liver
- • Pregnant
- • Incarcerated
- • New hemodialysis
- • Blood pressure \< 90/60, Pulse \> 120, O2 \> 6L or \>2L above baseline
- • HIV+/CD4 count \< 200
- • Receiving hospice services
- • Concurrent enrollment in a related research study
