ClinConnect Summary

This clinical trial is looking at a new digital health program called PREVENT, designed to help people with overweight or obesity improve their heart health and lifestyle choices. The study will involve 100 patients from rural primary care clinics, specifically examining how well the program works and how easy it is to use. Researchers believe that PREVENT can motivate patients to make healthier choices and improve their overall health.

To be eligible for the trial, participants need to be between 18 and 64 years old, have a low income (earning less than double the federal poverty level), and have a body mass index (BMI) of 30 or higher, which means they are classified as overweight or obese. Participants will receive support and counseling through the PREVENT program, and their experiences and health outcomes will be monitored throughout the study. This research is still in the planning stage and has not started recruiting participants yet, but it aims to make healthcare more effective for those at risk of heart disease.

Eligibility criteria

  • Patient Inclusion Criteria:
  • Aged 18-64 years at baseline
  • Low income (household income \<200% FPL)
  • At risk for poor CVH (body mass index greater than or equal to 30)
  • Receiving care from the Missouri Highlands Healthcare
  • Ability to understand and willingness to sign an IRB approved written informed consent document (or that of legally authorized representative, if applicable)
  • Provider Inclusion Criteria:
  • • All providers and clinic staff (physicians, nurses, community health workers, clinic staff, clinic research associates) in the Missouri Highlands Healthcare Clinics are eligible to participate.
