ClinConnect Summary

This clinical trial is focused on improving how we identify and manage functional impairments and frailty in older Veterans. Functional impairment refers to difficulties in performing everyday activities, which can affect quality of life and independence. The goal of the study is to implement a new, simpler way for doctors in Veterans Health Administration (VA) primary care settings to measure and understand the functional status of older Veterans. By doing this, the trial hopes to better identify those who need support and create more effective care plans for them.

To participate in this trial, individuals must be Veterans who are at least 60 years old and currently receiving care at a VA primary care facility. If you join, you can expect to help shape a new approach that could enhance how healthcare providers assist older Veterans with their daily activities and overall well-being. It’s important to note that the trial is currently recruiting participants, and it aims to gather valuable data that will help improve long-term services and support for older Veterans in the future.
