This clinical trial is looking at ways to help people who have had a stroke become more physically active. The main goal is to see if a special program that includes personal coaching and tailored activity plans can encourage stroke survivors to spend more time doing moderate to vigorous physical activities, like brisk walking or swimming. Participants will be divided into two groups: one group will receive the new program, which includes up to six sessions of personal counseling and a customized activity plan, while the other group will only get standard information about physical activity. The study will last for six months, with check-ins at the beginning, three months, and at the end.
To be eligible for this study, participants must be at least 21 years old, have had a stroke at least six months ago, and be able to walk independently, even if they use a walking aid for short distances. They should also be living at home and have the support needed to engage in the program. However, people living in care homes or those with serious health issues that could prevent them from exercising will not be able to join. This trial offers a supportive way for stroke survivors to improve their physical activity and overall health, and it's an exciting opportunity for those looking to enhance their recovery.

Eligibility criteria
- Inclusion Criteria:
- • Aged 21 years and over
- • Haemorrhagic or ischaemic stroke
- • At least 6 months post-stroke
- • Living at home
- • Able to walk independently +/- gait aid short distances (self-reported)
- • Have sufficient cognition and/or carer support to engage in the interventions (clinical judgement)
- • Agreement to undertake a supported self-management physical activity intervention programme
- Exclusion Criteria:
- • Living in residential care
- • Unstable cardiac conditions, severe arthritis, or other conditions limiting the ability to exercise
- • Advised by GP/consultant not to undertake moderate-intensity exercise for health reason
- • Currently already participating in physical activity that meets recommended levels (150mins of moderate-vigorous physical activity)
- • Enrolled in another physical activity intervention trial
