This clinical trial is studying a pain relief technique called an Intermediate Cervical Plexus Block, which is being used alongside general anesthesia for patients having neck surgeries, such as total thyroidectomy or total laryngectomy. The researchers want to see how effective this method is by testing two different concentrations of a medication called bupivacaine, which helps numb the area and reduce pain after surgery.
To participate in this study, you would need to be at least 40 years old and be in generally good health (with some specific health criteria). People with certain heart, lung, liver, or kidney problems, or those with neck infections or rashes, cannot join. If eligible, you'll receive the pain relief treatment during your surgery and will be monitored closely to assess how well it works in managing your pain afterward. This research aims to improve pain management for future patients undergoing similar surgeries.

Eligibility criteria
- Inclusion Criteria:
- • Any patient aged ≥ 40 years
- • ASA physical status I-III scheduled for total thyroidectomy or total laryngectomy under GA.
- Exclusion Criteria:
- • Patients with heart failure
- • history of arrhythmias
- • treatment with antiarrhythmic drugs
- • impaired pulmonary
- • Liver or kidney functions
- • Neck infection or rash
