ClinConnect Summary
This clinical trial is looking at how teamwork among healthcare professionals can help reduce unnecessary hospital visits for older adults. The idea is that when doctors, nurses, and other health workers collaborate closely, they can provide better care and keep patients healthier at home. Previous studies suggest that this teamwork can lead to fewer hospital stays and better health outcomes, especially for seniors who may face risks when hospitalized.
To participate in the trial, you need to be at least 75 years old, or 65 if you have chronic health problems, and you must have a general practitioner in the Pays de la Loire region of France. This study will focus on patients who have already had at least one visit with their doctor. If you join, you’ll be part of a program designed to improve your care and potentially help keep you from being admitted to the hospital. It's important to note that this trial is in the planning stages and has not started recruiting participants yet.
