This is an observational two-year study in Barcelona called K-HEALTH in AIR – Barcelona Pilot. It follows about 200 people who have chronic lung conditions (mainly severe asthma or COPD) and several other health problems. The goal is to see how the air inside people’s homes (indoor air quality) relates to health events like flare-ups and hospital visits, and to test a new integrated care approach that combines lung tests, home air-quality monitoring, digital support for self-care, and a computer model to predict when problems might happen. The study will collect information from home monitors, lung tests, questionnaires, and a mobile health app, with the aim of improving care and reducing unplanned hospital care, ER visits, and other health costs.
Eligible participants include adults and some older youths up to 85 years old who have COPD or treatment-resistant asthma and a high burden of other health problems, and who live in Barcelona (with some exceptions for asthma patients). Dementia or the inability to carry out daily activities exclude someone from joining. Participants will be followed for about two years, with all activities divided into two phases: Phase I (2024) tests the four components of the intervention, and Phase II (2025) evaluates the overall integrated care service and how it might be expanded, using a comparison group. Expect ongoing lung testing every six months, home air-quality monitoring, regular questionnaires, use of the Health Circuit app and a wearable, and nurse-supported care coordination. No results are available yet, and data sharing is limited.

Eligibility criteria
- Inclusion Criteria:
- • Aged Maximum 85 years
- • Diagnosed with chronic obstructive pulmonary conditions such as COPD or treatment-resistant asthma.
- • Exhibiting a high burden of co-morbidities, assessed above the 80th percentile of the regional risk stratification pyramid using Adjusted Morbidity Groups (AMG) scoring.
- • Residing in Barcelona-Esquerra, except for treatment-resistant asthma patients, live in any district of the city of Barcelona.
- Exclusion Criteria:
- • Dementia.
- • Inability to perform independent daily activities.
