This clinical trial is studying two different methods to help manage pain after knee surgery, specifically total knee replacement. In the trial, patients will be randomly divided into two groups. One group will receive a combination of two specific nerve blocks (called genicular nerve block and IPACK block) to help reduce pain, while the other group will receive a different nerve block (called adductor canal block) which will serve as a comparison. The effectiveness of these methods will be measured using a simple pain scale, where patients can rate their pain before and after surgery.
To be eligible for this trial, participants should be between the ages of 40 and 70 and in good overall health, meaning they are classified as ASA I to III, which refers to their physical status before surgery. They should be scheduled for a single knee replacement surgery and should not have any major health issues, such as liver or kidney problems, or a history of certain disorders. Participants can expect to undergo a spinal block for anesthesia during their surgery and will receive one of the pain management techniques being studied to help them feel more comfortable afterward. This trial is currently looking for volunteers to help find the best way to manage pain after knee surgery.

- Inclusion Criteria:
- • 1. Patients American Society of anesthesiologists' physical status (ASA) I to III.
- • 2. Aged 40 to 70 years.
- • 3. Both sexes.
- • 4. Patients scheduled for unilateral total knee replacement.
- Exclusion Criteria:
- • 1. Patients with spinal malformation
- • 2. Patients with liver impairment
- • 3. Patients with renal impairment
- • 4. Patients younger than 40 years or older than 70 years
- • 5. Patients with hypersensitivity to one of the used drugs
- • 6. Patients with neuromuscular disorders and
- • 7. Coagulopathy disorders
- • 8. Patients scheduled for revision knee arthroplasty or those who had a past history of previous surgery or trauma to the knee
