This clinical trial is looking at two different ways to manage pain for women undergoing major gynecologic surgeries. The researchers want to find out if combining an epidural with a specific type of pain block called a TAP block is more effective than using the epidural alone. They will compare how much pain participants feel and how much medication they need after surgery.

To take part in this study, participants must be women aged 18 and older who are scheduled for an elective major surgery related to gynecologic issues. They should be generally healthy but should not have certain conditions like a history of chronic pain or allergies to local anesthetics. During the trial, participants will receive general anesthesia for their surgery, and they will be randomly assigned to either receive the combined treatment or just the epidural. After the surgery, the researchers will evaluate pain levels and medication usage to see which method works better.

Eligibility criteria

  • Inclusion Criteria:
  • Elective major laparotomy gynaecology procedure.
  • Age 18 years and above
  • ASA I, II, III
  • Exclusion Criteria:
  • Prolonged INR
  • Allergic to LA
  • History of chronic pain
  • Psychological disorder/addict to opioid or benzodiazepine
  • Any contraindication for epidural or TAP block procedure
  • Pregnancy
