This clinical trial is focused on understanding how common a condition called Normal Pressure Hydrocephalus (NPH) is in Baltimore and Maryland, especially within the Black community. NPH is a condition that can affect brain function and is often overlooked. The study aims to find out if there are gaps in healthcare access based on where people live and their race. To help with this, the researchers will provide education about NPH to patients, their primary care doctors, and community health workers. They will also track how many people are referred to specialists and how often they use healthcare services over time.
To be eligible for this study, participants need to be at least 65 years old, have completed a specific health survey, and live in Maryland. The study will also involve certified Community Health Workers and primary care doctors who treat patients in the Johns Hopkins system. While the study is not yet recruiting participants, it aims to identify and reduce racial disparities in access to care for NPH, which could lead to better treatment options for everyone in the community.

Eligibility criteria
- Inclusion Criteria:
- • People over 65 years old who have completed the Annual Wellness Survey (AWV) survey
- • have a clinical profile in the Hopkins Epic data sets
- • live in Maryland
- Exclusion Criteria:
- • People under 65 years old will be excluded if they have not completed the AWV survey
- • do not live in Maryland
- Community Health Worker (CHW) Inclusion Criteria:
- • certified Community Health Workers from Maryland
- • completed accredited training by the Maryland Department of Health
- Primary Care Physician inclusion criteria:
- • must have patients in Johns Hopkins University AWV
