This clinical trial is studying two different methods for cleaning dental implants that have become infected and are causing a condition called peri-implantitis, which involves serious bone loss and inflammation around the implant. The researchers will compare an air-polishing device and a titanium curette to see which method is more effective in cleaning these contaminated surfaces. After the cleaning process, the implants will be removed, and tests will be conducted to analyze how well each method worked.
To participate in this study, you need to be an adult between 18 and 80 years old and require the removal of at least one titanium implant due to severe peri-implantitis. It’s important that you haven't received any treatments for this condition in the last six months. Participants can expect to undergo a cleaning procedure followed by the removal of the implant, with the goal of helping improve future treatments for similar issues. Please note that certain health conditions or recent medical treatments may prevent you from being eligible for this trial.

Eligibility criteria
- Inclusion criteria:
- • Adult individuals between 18 and 80 years of age who require the explantation of at least one titanium or titanium alloy implant due to severe peri-implantitis (\> 50% bone loss and signs of inflammation);
- • Individuals who did not undergo surgical or non-surgical peri-implant therapy in the previous 6 months.
- Exclusion criteria:
- • Acute infection associated with adjacent teeth;
- • Any technical complication that does not allow implant removal using a reverse torque device;
- • Active infectious diseases of any kind;
- • Medical conditions which requires premedication prior to dental treatments/visits;
- • Pregnant women or planning to become pregnant (self-reported);
- • History of radiotherapy in the head and neck or chemotherapy in the last 3 years;
- • Any other diseases or medications that may contraindicate the surgical procedure or compromise wound healing.
