ClinConnect Summary
The PACT Study is looking at how a special health coaching program can help people with type 2 diabetes better manage their condition. This program involves working with trained health coaches who guide participants over three months to set personal health goals, understand their diabetes better, and improve their self-care habits. The main focus of the study is to see if this coaching approach leads to lower blood sugar levels, measured by a test called HbA1c, over time compared to those receiving standard care for diabetes.
To join the study, you need to be between 21 and 64 years old, have a diagnosis of type 2 diabetes, and have had a blood sugar level (HbA1c) above 8% in the last six months. The study is currently recruiting participants, and if you qualify, you will receive support through phone or messaging to help you stay on track with your diabetes management. This trial aims to find out if personal coaching can make a real difference in managing diabetes and improving overall health.
