The OPTIMISE trial is a study aimed at improving the treatment of heart and related health issues in individuals at high risk for cardiovascular disease (CVD). CVD is a major cause of death in the UK, but by managing risk factors like blood pressure, cholesterol, diabetes, and weight, we can significantly reduce these risks. This trial will compare two groups of patients: one group will receive extra consultations with heart specialists at their local GP practice, while the other group will continue with standard care from their regular doctors. The goal is to see if the extra support from specialists leads to better health outcomes.
To be eligible for this trial, participants need to be part of a related study called FIND-AF and have a higher risk score based on that study. Participants will be monitored over six months, during which they will have their medications reviewed and their health measurements, such as blood pressure and cholesterol levels, checked. Additionally, everyone will fill out a questionnaire about their quality of life to understand how their health may have improved. This study is not yet recruiting participants, but it aims to help improve care for those at risk of serious health issues.

- Inclusion Criteria:
- • Consented participants in the FIND-AF study
- • Higher predicted FIND-AF risk according to the FIND-AF score
- Exclusion Criteria:
- • Unable to give written informed consent for participation in the study
- • Unable to adhere to the study requirements
