This clinical trial is studying ways to prevent a condition called iatrogenic dependence linked to hospital stays in elderly patients. This condition is common in people over 75 years old and can happen due to factors like being immobile, falling, not eating enough, confusion, and side effects from medications. The trial aims to test a program called the TAKE CARE intervention, which provides healthcare staff with better practices to help reduce these risks in patients who are hospitalized for acute illnesses, not including those recovering from surgery.
To be eligible for this trial, participants need to be at least 75 years old and admitted to the hospital unexpectedly for an acute health issue. Those who have a very low ability to perform daily activities before their hospital stay, have been in another facility for more than 72 hours before admission, or do not want to participate cannot join. During the trial, participants will receive either the TAKE CARE intervention or the usual hospital care, and researchers will track their progress over six months, including interviews to understand how well the program works. This study is important because it could lead to better care for elderly patients in hospitals and help them recover more effectively.

Eligibility criteria
- Inclusion Criteria:
- • Age ≥ 75 ans
- • Unscheduled hospitalisation in one of the participating departments for an acute pathology, excluding post-operative care
- Exclusion Criteria:
- • Patients with a usual ADL score (assessed 15 days before hospitalisation) of 0
- • Stay \> 72 hours in another facility before admission to UGA
- • Opposition to participation in the study (trusted support person / guardian - curator)
- • Not affiliated to a social security scheme (beneficiary or beneficiary entitled)
- • Patient under AME
