The BECOME study is a clinical trial focused on improving mental health and managing chronic diseases in Nepal. It is looking at how community health workers can help people aged 40 and older who have mental health disorders like anxiety or depression, along with non-communicable diseases (NCDs) such as high blood pressure or diabetes. The trial will involve about 600 participants who will receive support through three proven behavioral approaches aimed at reducing stress, enhancing mood, and encouraging healthier lifestyle choices. Participants may be eligible if they live in the target areas, have a diagnosis of anxiety or depression, and also have at least one NCD.
If you decide to participate, you'll be working with trained community health workers who will provide support over a year. The study will gather information on how well these interventions work and how they can be delivered effectively in local communities. Additionally, the research team will also collect feedback from participants and health workers to understand the impact of the program. This study could pave the way for better mental health and chronic disease management in places with limited healthcare resources.

Eligibility criteria
- Inclusion Criteria:
- • Adult men and women age ≥ 40 years
- • Living in the target wards with no intention of leaving in the next 2.5 yrs
- • Anxiety (HSCL-25 anxiety subscale score ≥1.75) and/or Depression (HSCL-25 depression subscale score ≥1.75)
- • At least one Non-communicable disease (NCD) based on WHO PEN criteria; either Hypertension (HTN) (SBP ≥130mmHg and/or DBP ≥80mmHg) and/or Diabetes Mellitus (DM) (fasting plasma glucose (FPG) ≥126mg/dl or random plasma glucose ≥200mg/dl)
- Exclusion Criteria:
- • Significant cognitive problems/disability
- • Pregnant women
- • Postpartum (≤6 weeks) women
