This clinical trial, titled "Prioritising Patient Medication Review: Hospitals Reaching Out," is studying how a comprehensive review of medications can help older patients who are taking many different medicines (known as polypharmacy) when they are admitted to the hospital. The trial focuses on patients aged 65 and older who are in two specific places: an acute admission unit (where patients stay for less than 48 hours) and a medical outpatient setting (where patients come for regular check-ups or treatments). The goal is to see if this medication review can improve the overall health and safety of these patients.

To be eligible for the study, patients must be hospitalized and prescribed at least seven different medications when they are admitted. Unfortunately, patients who are expected to have a very short life left, those living outside the region, or those who do not speak Danish will not be included. If you or a loved one qualifies, participating in this trial means you will receive thorough attention to your medications from a team of healthcare professionals, helping to ensure that all medications are necessary and safe for you. The trial is currently recruiting participants, and it aims to improve communication about medications between different healthcare providers.

Eligibility criteria

  • Inclusion Criteria:
  • all hospitalized patients who are prescribed at least seven medications specified in the Electronic Patient Journal (EPJ) at admission
  • Exclusion Criteria:
  • terminal patients or patients with a short life expectancy, patients residing in another region, patients who are discharged on the day of inclusion, and finally patients who do not speak Danish.
