This clinical trial is focused on improving the care and outcomes for patients who have experienced an ischemic stroke, which is a type of stroke caused by a blockage in the blood vessels supplying the brain. The researchers want to see if using a special management system that helps doctors make better decisions can lead to better recovery for patients after a stroke. This study will involve comparing different groups of patients to assess how well this system works in enhancing patient care.

To be eligible to participate, you need to be at least 18 years old and have been diagnosed with an ischemic stroke confirmed by brain imaging tests like MRI or CT scans. You should also be within 7 days of experiencing symptoms and have a certain level of stroke severity when admitted to the hospital. However, individuals with other types of cerebrovascular diseases or certain health conditions won’t be able to take part. If you join the trial, you can expect to receive personalized care from healthcare professionals and your progress will be monitored closely to see how well the new management system helps your recovery.

Eligibility criteria

  • Inclusion Criteria:
  • Age ≥ 18.
  • Diagnosed with ischemic stroke confirmed by magnetic resonance imaging (MRI) or brain computed tomography (CT).
  • Within 7 days from onset of symptoms.
  • With a NIHSS score of ≥2 at admission (limb score ≥1).
  • With a prestroke mRS score of 0-1.
  • Admission from the emergency department or outpatient service.
  • Voluntary informed consent.
  • Exclusion Criteria:
  • Diagnosis of other cerebrovascular diseases (cerebral hemorrhage, subarachnoid hemorrhage, cerebral venous sinus thrombosis, etc.).
  • Diagnosis of non-cerebrovascular disease (central nervous system infection, epilepsy, metabolic encephalopathy, etc.).
  • Ischemic stroke with negative diffusion weighted imaging (DWI).
  • Being tested for interventions such as drugs or instruments.
  • Pregnancy or 6 weeks postpartum.
  • With a life expectancy of less than 3 months or who were unable to complete the study follow-up for other reasons.
