This study is testing whether adding ELDOA exercises to a standard upper‑back treatment plan helps people with thoracic hypomobility (stiffness in the mid‑back) and posture problems like forward head posture and rounded upper back. Participants are randomly assigned to two groups: one gets thoracic manipulation and regular physical therapy (MAN group), and the other gets the same care plus ELDOA exercises (ELDOA‑MAN group). The trial is designed so the investigator doesn’t know which group a participant is in to reduce bias.
Eligible participants are adults aged 19 to 44 of any gender who have upper back pain, forward head posture, thoracic hypomobility, and increased thoracic kyphosis. Exclusions include conditions such as certain spine diseases, prior spine surgery, scoliosis, spinal stenosis, spinal TB, or fibromyalgia, among others, and they must be able to attend regular therapy sessions. The study plans about 40 participants and is enrolling by invitation at Foundation University Islamabad in Pakistan. Primary outcomes are measured over about 3 weeks and include pain on a 0–100 scale, kyphosis angle, thoracic range of motion, forward head posture, and a functional ability score. Results aren’t available yet since the trial is ongoing.

- Inclusion Criteria:

- • Upper back pain or discomfort
- • Forward head posture
- • Thoracic hypomobility
- • Thoracic hyperkyphosis
- Exclusion Criteria:
- • Ankylosing spondylitis
- • History of Spinal Fusion or spinal surgery (discectomy/laminectomy etc.)
- • Degenerative disc disorders, Facet joint syndrome
- • Cervical radiculopathy, Scoliosis
- • Spinal Stenosis or Myelopathy
- • Recent fracture or surgery
- • TB of spine, Spondylolisthesis
- • Not able to attend regular therapy appointments
- • Fibromyalgia/myofascial pain syndrome
