This clinical trial is looking at a new exercise program designed for people with axial spondyloarthritis (axSpA), a type of inflammatory arthritis that primarily affects the spine. The study will run for six months and includes two phases: participants will start with a three-month exercise program that includes high-intensity interval training, strength exercises, and aerobic activities. There will also be short videos to help patients understand their condition better and motivate them to stay active. Participants will use a special app called SPARK, which will guide them through the exercises and track their progress.
To join this trial, participants should have been diagnosed with axSpA in the last two years and currently experience active symptoms. However, certain health conditions, such as active eye inflammation or serious diseases like cancer, may prevent someone from participating. Throughout the trial, participants can expect personalized support through the app to help them stay on track with their exercise routine. This study aims to see if this digital exercise program can improve disease activity and overall health for those living with axSpA.

Eligibility criteria
- Inclusion Criteria:
- • axSpA diagnosis within the last two years by a rheumatologist according to the ASAS criteria of 2009
- • active disease (ASDAS \> 1.3)
- Exclusion Criteria:
- • Active uveitis.
- • CRP \> 30.
- • Former or current use of bDMARDs.
- • Pregnancy or planned pregnancy within 6 months from inclusion.
- • Absolute and relative contradictions to high intensity exercise according to the American College of Sports Medicine (ACSM)
- • Other serious disease such as cancer.
- • Patients incapable to follow the protocol or the control set-up with remote monitoring.
