This clinical trial is studying the challenges some patients face when trying to stop using a breathing machine called mechanical ventilation. Specifically, it looks at how different levels of breathing effort can affect the recovery process for patients who have been on this machine. The researchers want to find out if high levels of breathing effort are linked to difficulties in stopping the use of the machine, as well as longer hospital stays. This study will take place in two hospitals from January 2024 to July 2026 and involves patients over 18 years old who are on mechanical ventilation and are starting to breathe on their own.
To be eligible for the trial, patients must be admitted to the hospital, be over 18, and have been on mechanical ventilation for less than 7 days. However, those with certain conditions, like severe lung diseases or who are pregnant, cannot participate. If a patient joins the study, they will be closely monitored during the first week of their recovery to gather information about their breathing efforts and overall progress. This trial aims to better understand how to help patients wean off mechanical ventilation more safely and effectively.

Eligibility criteria
- Inclusion Criteria:
- • Patients over 18 years old, admitted to the Glória D'or hospital, undergoing orotracheal intubation and invasive mechanical ventilation, in their first 7 years on spontaneous ventilation, who accept to participate in the study by signing the free and informed consent form (or by their legal responsible).
- Exclusion Criteria:
- • • Neuromuscular disease;
- • Phrenic nerve injury;
- • MV for less than 24 hours;
- • Previously tracheostomized patient;
- • Age under 18;
- • Use of neuromuscular blocker \> 72h;
- • COPD;
- • Patients with pulmonary fibrosis;
- • Pregnancy;
- • Patient under palliative care;
- • COVID-19 pneumonia;
- • Patients reintubated after extubation in this hospitalization for less than 3 weeks.
