This clinical trial is studying whether a supplement called nicotinamide, which is a form of vitamin B3, can help improve physical performance in older adults who are at risk of falling. Falls can be very serious for people over 65, leading to injuries and a loss of confidence. The trial involves participants aged 65 and older who have experienced multiple falls in the past year, have balance issues, or have certain scores indicating a risk of falls. However, those with specific muscle diseases, severe joint pain that affects exercise, or cognitive issues that prevent them from understanding the study cannot participate.
Participants in this trial will take nicotinamide while also engaging in a physical training program designed to strengthen muscles and improve balance. The goal is to see if this combination can help them become more physically capable and reduce their risk of falls. This research could lead to new ways to help older adults stay active and safe as they age.

Eligibility criteria
- Inclusion Criteria:
- • Aged 65 years or older
- • Living in the community
- * Increased risk of falls, defined as:
- • More than two falls in the last 12 months or
- • Altered balance test (unable to maintain unipedal stance \> 5 seconds) or
- • SPPB score less than 10 points
- Exclusion Criteria:
- • Known Muscle Disease (dystrophy, myopathy, or similar)
- • Motor sequelae of stroke
- • Joint pathology with severe pain preventing strength exercises
- • Cognitive impairment that renders the informed consent process impossible.
