ClinConnect Summary
This clinical trial is looking at how a mobile health app can help older adults with diabetes manage their condition more effectively. It builds on previous findings that having nurses provide proactive consultations can benefit patients, but this approach can be time-consuming and requires many resources. The goal of this trial is to see if using technology, like a smartphone app, can offer a more convenient way for patients to take control of their diabetes care.
To participate in the trial, you need to have been diagnosed with diabetes for at least one year and be using a government-approved mobile health app. You also need to live in an area where you have access to the internet. Unfortunately, if you are already part of another mobile health program or have certain physical or mental limitations, you may not be eligible. Those who join the trial can expect to receive support through the app to help manage their diabetes, making it easier to track their health and make informed decisions.
