This clinical trial is studying a new treatment method called Cardioneuroablation (CNA) for patients who experience severe fainting spells, known as reflex syncope. The goal is to find the best ways to identify specific areas in the heart that need to be treated to help prevent these fainting episodes. The study will compare three different techniques to see which one accurately locates these areas, helping doctors improve the CNA procedure for better patient outcomes.
To participate in this trial, patients must have experienced serious fainting spells linked to a slow heart rate, especially if these spells have caused injuries or have not improved with other treatments. Participants should also have shown a positive response to a specific heart test involving a medication called atropine. The study is open to adults of all ages, and participants will undergo standard CNA procedures while doctors test the three methods for locating the treatment sites. It's important to note that patients who cannot safely undergo CNA or a CT scan with contrast cannot join the study.

Eligibility criteria
- Inclusion Criteria:
- • severe, recurrent symptoms due to reflex syncope with ECG documented asystole \>3 seconds, especially if associated with injury, or recurrent presyncope with persistent reflex bradycardia
- • history of ineffective prior non-pharmacological treatment and positive baseline atropine test (sinus rate acceleration \> 30% and no AV block following 2 mg of intravenous atropine) -
- • informed written consent obtained
- Exclusion Criteria:
- • contraindications to perform cardioneuroablation
- • contraindications to perform computed tomography with intravenous contrast injection
- • lack of informed written consent obtained
