This clinical trial is studying the effects of time-restricted eating on pregnant women who are severely obese. Time-restricted eating means that participants will only eat during certain hours of the day, which may help manage their weight and improve health outcomes for both the mother and baby. The trial is important because severe obesity can lead to serious health issues during pregnancy, such as gestational diabetes and high blood pressure, particularly affecting Black women more than others.

To be eligible for this study, participants should be females aged 18 to 44 with a body mass index (BMI) between 40 and 50, who are pregnant with one baby and less than 17 weeks along. Participants will need to speak English, be able to give consent, and have access to a smartphone for the study. Throughout the trial, those who join can expect regular check-ins and guidance on their eating schedule, while also receiving standard prenatal care. It's a unique opportunity to potentially improve health during pregnancy while contributing to important research that could help future mothers and babies.

Eligibility criteria

  • Inclusion criteria:
  • Female based on sex assigned at birth
  • Preconception body mass index (BMI) 40.0 - 50 kg/m2
  • Singleton pregnancy
  • Age 18-44 years old
  • \< = to 17 weeks gestational age
  • Fluency in English to provide consent and complete study procedures
  • Ability to provide informed consent
  • Cleared by study doctor and the obstetrician/mid-wife provider to participate
  • Access to a smartphone to complete intervention procedures
  • Exclusion criteria:
  • Deemed medically high risk
  • Multiple pregnancy (e.g., twins)
  • Type 1 or 2 diabetes mellitus
  • Early gestational diabetes (diagnosed at \< = to17 weeks gestational age through an oral glucose tolerance test)
  • Currently eating ≤ 12 hours daily
  • Autoimmune disorder (e.g., rheumatoid arthritis)
  • iron deficiency anemia
  • Inflammatory bowel disease
  • Previous spontaneous preterm birth
  • History of bariatric surgery
  • Night shift work
  • Currently incarcerated
