This clinical trial is designed to compare two medications, fentanyl and morphine, to see which one works better in helping patients who are on a ventilator in the Intensive Care Unit (ICU) feel comfortable and relaxed. The main goal is to find out if patients receiving fentanyl can achieve the desired level of sedation (or sleepiness) more effectively than those getting morphine over 12 and 24 hours. The study will also look at whether patients in one group need extra doses of medication to help them relax compared to the other group.
To participate in this trial, patients must be over 18 years old and expected to need mechanical ventilation for more than 24 hours, along with receiving sedation. However, patients who have a history of long-term narcotic use, severe liver or kidney failure, certain neurological conditions, or are pregnant, among other exclusions, will not be eligible. If someone qualifies and chooses to participate, they will help researchers learn more about effective sedation methods in critically ill patients, which could improve care in the future.

Eligibility criteria
- Inclusion Criteria:
- • Age \>18 years old
- • Patients who are expected by the ICU team to require invasive mechanical ventilation in ICU for at least more than 24 hours and for whom the ICU team decides to initiate continuous infusion of sedation
- • A patient who was ventilated and sedated not more than 12 hours before ICU admission.
- Exclusion Criteria:
- • Patients with chronic use of narcotics
- • Patients with chronic liver failure or end-stage renal failure
- • Patients with severe chronic neurocognitive dysfunction
- • Patients with drug overdose
- • Patients with a known allergy to either morphine or fentanyl
- • Patients who are receiving neuromuscular blockers
- • Patients who are pregnant
- • Patients who are diagnosed to have severe traumatic brain in
