This clinical trial is studying how two different types of mechanical ventilation (breathing support) affect patients undergoing laparoscopic gynecologic surgery, particularly when the patients are positioned in a way that can impact their breathing and blood flow. The researchers want to see if using one ventilation method over the other helps with breathing and brain oxygen levels after surgery, especially since some patients can develop breathing problems known as atelectasis.
To participate in this study, women aged 18 to 70 who are generally healthy (classified as ASA 1-3, meaning they have minimal to moderate health issues) may be eligible. However, those with more serious health conditions, such as severe lung disease or certain heart problems, will not qualify. If you join the trial, you can expect to be monitored closely for how well you breathe and how your body responds during and after the surgery. It's also important to know that your consent is needed to participate, ensuring you understand what the study involves before agreeing to take part.

Eligibility criteria
- Inclusion Criteria:
- • 18-70 age
- • ASA 1-3
- Exclusion Criteria:
- • Under 18 years of age, over 70 years of age,
- • ASA IV,
- • history of severe chronic obstructive pulmonary disease (COPD, GOLD III or IV)
- • ,-history of severe or uncontrolled bronchial asthma, presence of restrictive lung disease,
- • history of any thoracic surgery operation, need for thoracic drainage before surgery
- • patients receiving preoperative renal replacement therapy,
- • congestive heart failure (NYHA grade III or IV),
- • extremely obese (body Mass Index, BMI \> 35 Kg/m2)
- • patients without patient consent
