The PT4A clinical trial is studying a new way to help people with high blood pressure, also known as hypertension, in western Kenya. The researchers want to see if having peers—people from the community—deliver medications directly to patients can improve how well they stick to their treatment plans. They will also use technology to support this process, like providing advice over the phone and coordinating care between doctors and pharmacies. Early results suggest that this method may help patients take their medications more regularly and lower their blood pressure, but the researchers need to gather more evidence to confirm these findings.
To participate in this trial, you need to be an adult enrolled in AMPATH's Chronic Disease Management Program and have high blood pressure that is not well controlled. This means your blood pressure readings are higher than the normal levels. Unfortunately, if you are experiencing a medical emergency or have a serious illness that requires immediate attention, you won’t be eligible for this study. If you join, you can expect to receive medication delivered to your home by a peer, and you will have access to support through phone consultations. The goal is to make it easier for patients to manage their health and improve their overall well-being.

Eligibility criteria
- Inclusion Criteria:
- • • Adult participants enrolled in AMPATH's CDM Program with uncontrolled hypertension (SBP ≥ 140 or diastolic BP (DBP) ≥ 90)
- Exclusion Criteria:
- • hypertensive emergency requiring immediate medical attention,
- • terminal illness, and
- • inability to provide informed consent.
