This clinical trial is studying a lifestyle program designed to help people with diabetes prevent the return of foot ulcers, which are painful sores that can lead to serious complications, including amputation. Many individuals who have these ulcers struggle to maintain the necessary care routines, such as wearing special shoes that relieve pressure on their feet, even after their ulcers have healed. The goal of this study is to see if a supportive program can help participants develop daily habits that make it easier to take care of their feet and manage their diabetes effectively.
To join this trial, participants must be at least 21 years old and have had a diabetic foot ulcer that has healed within the past year. They should be able to walk independently, although using a cane is okay. However, individuals with an active foot ulcer or who have had a more significant amputation are not eligible. If you join the study, you can expect to participate in activities that encourage healthier habits to help prevent foot ulcers from coming back.

Eligibility criteria
- Inclusion Criteria:
- • Age \>= 21 years
- • Healed diabetic foot ulcer within the past twelve months
- • Able to walk independently (use of a cane is acceptable)
- Exclusion Criteria:
- • Current active diabetic foot ulcer
- • History of amputation beside toe or part of the foot
- • Non-ambulatory at baseline
- • Self-reported deafness/blindness.
