The DEDICATE trial is studying how to improve the way health care teams in community health centers can help patients with social needs, such as housing and food insecurity, in addition to managing health conditions like diabetes and high blood pressure. The goal is to see if providing support to these teams will help them use electronic health records (EHR) more effectively to identify and connect patients with community resources that can assist with their social needs. This study will take place in several health centers and will measure whether the teams can increase their use of these tools to screen for social needs and refer patients to helpful organizations.
To participate, health centers need to use an EHR tool to manage care for at least 15 patients each month who have social needs. However, health centers that have already tested this program, school-based health centers, or those serving prison populations are not eligible. While the trial is not yet recruiting participants, the findings could greatly benefit patients by improving the support they receive in managing both their health and social needs in primary care settings.
