This clinical trial is studying a new surgical technique called cross-leg vascularized fibular graft to help patients who have suffered large bone defects in their tibia (the larger bone in the lower leg) due to injury. The goal is to see if this method can restore the normal shape and function of the injured leg. The study will take place at Assiut University Hospital's Orthopedics and Trauma Surgery department, and it aims to help people who have significant bone loss (between 12 to 26 cm) along with serious damage to surrounding soft tissues and limited blood supply.
To participate in this trial, patients must be admitted to the hospital with these specific tibial issues, but those with similar problems in both legs or certain medical conditions that prevent them from safely undergoing long surgeries will not be eligible. While the trial is not recruiting participants yet, those who do join can expect to receive this innovative grafting procedure aimed at improving their leg's function and overall quality of life.

Eligibility criteria
- Inclusion Criteria:
- • - Patients who were admitted into Assiut University Hospital, Orthopedics and Trauma Surgery department with big tibial bone defects ranging from 12 cm to 26 cm, associated with extensive soft tissue damage and poor ipsilateral blood supply confirmed by CT angiography and treated with cross-leg free vascularized fibular graft.
- Exclusion Criteria:
- • Patients with bilateral extensive soft tissue damage and poor blood supply.
- • Patients with medical conditions preventing them from general anesthesia for long periods (\>10 hours).
