This clinical trial is studying a method of helping patients breathe better while they are on a ventilator, specifically looking at a technique called pressure support ventilation (PSV). PSV provides extra pressure to assist patients who are having trouble breathing due to a condition called acute hypoxemic respiratory failure, where their lungs can’t get enough oxygen. The aim of the study is to see if measuring something called the pressure-muscle-index (PMI) can help doctors provide the right amount of breathing support, avoiding too much or too little help, which can aid in a quicker recovery and help patients get off the ventilator sooner.
To participate in this trial, patients must be over 18 years old, currently receiving invasive ventilation in an Intensive Care Unit, and diagnosed with acute hypoxemic respiratory failure (indicated by specific blood oxygen levels). Patients with certain medical conditions or those who are too agitated may not be eligible. For those who qualify, the study will involve monitoring their breathing support closely to find the best settings for their recovery. The trial is not yet recruiting participants, but it aims to provide valuable insights into improving care for patients needing ventilator support.

Eligibility criteria
- Inclusion Criteria:
- • \>18 y.o.
- • Invasively ventilated in Intensive Care Unit.
- • Diagnosed with acute hypoxemic respiratory failure (P/F\<300 mmHg).
- • Breathing on pressure support ventilation.
- Exclusion Criteria:
- • \<18 y.o.
- • Contraindication for Electrical Impedence Tomography monitoring (e.g. pacemaker, burns, or - wounds limiting electrode placement).
- • Severe and previously diagnosed neurological and neuromuscular disease.
- • Psychomotor agitation (Sedation Agitation Scale \> 4).
