ClinConnect Summary
This clinical trial is looking to find out if providing extra support through navigation and text messages can help people join and stay engaged in a program called "Food is Medicine," which focuses on healthy eating for heart health. The study will involve individuals who are currently in cardiac rehabilitation. Participants will be randomly placed into one of four groups: one receiving navigation support, another receiving text messages, one receiving both, and a control group receiving neither. Everyone will fill out surveys at the beginning and after three months, and researchers will see how many healthy meals or groceries each group received during that time.
To be eligible for this study, you must be at least 18 years old, currently participating in UCSF Cardiac Rehabilitation, and able to communicate in either English or Spanish. You also need to live in San Francisco County. However, if you're in hospice care or unable to give consent for yourself, you won't be able to participate. This trial is currently recruiting participants, and it aims to understand how support affects healthy eating and overall engagement in the program.
