This study is testing whether a brief counseling method called 5A-5R, used in primary care clinics, can help adults stop using tobacco in rural Bangladesh. It compares three groups at three community clinics: (1) standard 5A-5R counseling with educational materials, (2) the same counseling plus weekly mobile phone calls and text messages for extra support, and (3) usual care with no structured counseling. About 714 adults who currently use tobacco will be enrolled and followed for 8 months. Because it uses a cluster design, entire clinics are assigned to one group, and the people collecting the outcome data don’t know which group a participant is in. Results aren’t available yet.
If you participate, you would meet with trained health workers at a local clinic. In the phone-support group, you’d also get weekly calls and texts to encourage quitting. You would fill out questionnaires about your tobacco use, whether you quit, and your knowledge and attitudes about tobacco at the start and around 8 months. Eligibility includes adults aged 18 or older who have used tobacco daily for the past six months and are willing to participate; exclusions include temporary residence at the study site, terminal illness, or significant mental health problems. Primary outcome is whether you have stopped using tobacco by 8 months; secondary outcomes include reduction in cigarettes per day, reduction in smokeless tobacco use, and any changes in knowledge or attitudes toward quitting. This trial is led by BRAC University with partners, and after the study, control sites will receive the counseling materials as well.

- • Adults (age 18 and above) having a history of current tobacco consumption/use (daily tobacco use during the last six months).
- • Willing to participate.
- Exclusion Criteria:
- • temporary resident at the selected study site.
- • having a terminal illness.
- • mentally unstable.
