This clinical trial is studying the use of sildenafil, a medication often used to treat erectile dysfunction, to see if it can help reduce problems in blood vessels for patients who are receiving support from a left ventricular assist device (LVAD). The LVAD is a mechanical pump used to help people with severe heart failure. While this support can improve survival, many patients experience rapid blood vessel aging, which can lead to serious issues like strokes and bleeding. The researchers hope that sildenafil's properties can help improve the health of these blood vessels during LVAD treatment.
To participate in this study, you need to be at least 18 years old and either currently using a durable LVAD or planning to get one. You also need to be able to provide informed consent, which means you understand the study and agree to take part. However, individuals with certain conditions, such as allergies to sildenafil or specific heart surgeries, cannot join. If you decide to participate, you can expect to be closely monitored as part of the trial, helping researchers learn more about how sildenafil can potentially benefit patients using LVADs.

Eligibility criteria
- Inclusion Criteria:
- • Over 18 years of age
- • Supported by a durable LVAD or planned to undergo placement of a durable LVAD
- • Be able to give informed consent
- Exclusion Criteria:
- • History of pre-existing aortic valve prosthesis or an aortic graft
- • Allergy to sildenafil
- • Taking any nitric oxide (NO) donor medications
- • History of complete carotid occlusion
